What Is a Fixed Orthodontic Appliance?
Braces treatments are fixed orthodontic methods in which teeth are gradually moved with brackets bonded to the teeth and archwires. Metal, ceramic (aesthetic) or lingual (tongue-side) bracket options may be assessed according to clinical findings and patient preference.
Fixed appliances are widely used for occlusal problems such as crowding, spacing, overjet and overbite. Whether extraction is needed is decided after examination, model analysis and imaging for each case; extraction is not a standard step in every orthodontic plan.
Examination and Treatment Plan
At the first orthodontic assessment, intraoral examination, panoramic X-ray and, if needed, cephalometric analysis are performed. Tooth-bone relationships, growth pattern and periodontal status guide the plan. Patient and family expectations are discussed before treatment.
Treatment duration depends on case complexity, patient compliance and growth factors; a fixed number of months is not promised in advance. Early assessment may make later treatment easier in some children; active bracket treatment is not started immediately in every child.
Bracket and Wire Process
Brackets are bonded to the tooth surface with a special adhesive; archwires pass through bracket slots to provide activation. At monthly or planned review intervals, wire changes, elastic use or additional appliances may be assessed.
Sensitivity or pressure lasting a few days may occur after wire change. Broken brackets or wires can occur after sticky foods or trauma; an urgent review appointment may be planned.
Oral Care and Reviews
Plaque accumulation around brackets increases the risk of caries and gum inflammation. Orthodontic brush, interdental brush and dental floss or water flosser use are recommended. Fluoride toothpaste and, if needed, fluoride mouthwash may be considered on clinician advice.
Keeping regular review appointments is important for treatment progress. When active treatment is completed, the retention stage starts; this period is not a continuation of active movement.
Difference from Clear Aligners
Fixed bracket treatment is worn continuously; patient compliance depends on the appliance in the mouth. Clear aligner systems are removable and intended for different case profiles. One method cannot be generalised as superior to the other; the choice is made according to clinical need.
In cases requiring complex rotation, vertical control or jaw relationship correction, fixed appliances may be preferred. After orthodontist examination, the suitable method is assessed together.
